 Post subject: Adding an authorized representative

While reviewing and updating my will and other financial information, I just realized that I never set an authorized representative with my Medicare. I'm thinking that it might be a good idea to have my son be able to speak for me and deal with things should I become incapacitated. How do I add an authorized representative now?

You can add an representative by filling the change of information form. In which you will see a part called "Addition/Deletion of Authorized Representative" for making changes on your authorized representative. The original signature of the new authorized representative
is required to add a new authorized representative.

I haven't begun thinking about who I want as my representative if my husband isn't around. I guess I will appoint my sons if the need arises.

I am just hoping I kick the bucket before my husband!

I believe you will need to print the form and have the person sign it and then mail it into the social security office to have it take effect. Not sure why that is but that's what a friend of mine had to do.
