My father lost his medicare card, how do we go about a getting a new one? Also, I think his address listed for Medicare was the one for our old house. Can I have that changed as well?

While you're at it, you may also request for a change of address using the sites given
or you may go to your local Social Security Administration office.

You will want to be sure to change your address before asking for a replacement card because they can only be sent to the address of record. Once you are sure the address is correct, then you can apply for a new card to be sent.
